Position Responsibilities
- Community Engagement: Build and maintain relationships with community members through regular outreach efforts. Identify community needs and challenges through engagement and assessment.
- Resource Coordination: Connect individuals and families with resources and services, including healthcare, education, housing, and employment opportunities. Assist with navigation of social services systems.
- Program Development: Collaborate with team members to develop, implement, and evaluate outreach programs designed to improve community wellbeing.
- Advocacy: Advocate on behalf of clients and the community for access to services and resources. Work to address systemic barriers to wellbeing.
- Education and Empowerment: Provide educational workshops and materials on various topics relevant to community needs. Empower individuals and groups to access resources and participate in their community.
- Collaboration: Work closely with other social workers, healthcare providers, and community organizations to coordinate services and support community health initiatives.

- Knowledge of local resources and social services systems.
- Ability to work flexibly and adaptively in various community settings.
- Commitment to ethical practice and continuing professional development.
