Position Responsibilities
Digital Health Advocacy: Champion the use of digital health tools and resources within community settings, educating clients on how these technologies can support their health and wellness goals.
Client Support: Provide direct support to clients, integrating digital health solutions into personalized care plans. Assist clients in navigating and utilizing digital health apps and platforms.
Program Development: Collaborate with the team to develop and implement programs that leverage digital health technologies to address the needs of diverse populations.
Community Engagement: Engage with the community through workshops, seminars, and events to promote awareness and adoption of digital health and wellness practices.
Collaboration: Work closely with healthcare providers, technology experts, and community organizations to enhance the accessibility and effectiveness of digital health resources.
Evaluation and Feedback: Gather and analyze feedback on the use of digital health tools among clients, contributing to the continuous improvement of services and resources offered.

Position Requirements
Bachelor’s or Master’s Degree in Social Work (BSW/MSW) from an accredited institution, with a valid state license.
Experience or strong interest in digital health, telehealth, or related fields.
Knowledge of current digital health trends, tools, and best practices.
Excellent communication and interpersonal skills, with the ability to engage effectively with a diverse range of clients and stakeholders.
Strong organizational skills and the ability to manage multiple projects simultaneously.
Commitment to ethical practice, ongoing professional development, and advocacy for health equity.
