Position Description
As a Nursing Home Social Worker, you will be an integral member of our multidisciplinary team, responsible for addressing the psychosocial needs of our residents. Your role will focus on supporting residents through individual counseling, group therapy, and the development of social programs that promote mental health and well-being. You will act as a liaison between residents, families, and the nursing home staff to ensure that our residents receive the highest level of care and support.

Position Responsibilities
Conduct comprehensive assessments of residents' social, emotional, and psychological needs.
Develop and implement personalized care plans, adjusting as necessary to meet the evolving needs of each resident.
Provide counseling and support to residents and their families, helping them navigate the emotional challenges associated with aging and living in a nursing home.
Facilitate group therapy sessions and social activities to encourage community and support among residents.
Collaborate with the healthcare team to advocate for the residents' needs and preferences.
Assist residents and their families in understanding and accessing community resources.
Maintain detailed records of interventions, resident progress, and any changes in mental health status.

Position Requirements
Bachelor's or Master's degree in Social Work (MSW preferred) from an accredited institution.
Valid state license in social work.
Previous experience working in a healthcare setting, with a preference for candidates who have experience in geriatric social work.
Strong interpersonal and communication skills, with a proven ability to build trust and rapport with diverse populations.
Knowledge of the social, emotional, and psychological challenges faced by the elderly.
Empathy, patience, and a deep commitment to enhancing the well-being of the elderly.
