Position Description
We are seeking a dedicated and compassionate Senior Social Worker to join our team at our nursing home facility. The ideal candidate will have extensive experience in social work within healthcare settings, particularly with elderly populations. The Senior Social Worker will play a crucial role in assessing the needs of our residents, developing care plans, and providing emotional support and counseling to both residents and their families. This role requires strong interpersonal skills, empathy, and the ability to work collaboratively with a multidisciplinary team.
Position Responsibilities

  1. Resident Care:

    • Conduct assessments and develop individualized care plans.

    • Provide counseling and emotional support to residents and families.



  2. Advocacy and Resources:

    • Advocate for residents' rights and coordinate with external agencies.

    • Ensure residents receive necessary services and support.



  3. Team Collaboration:

    • Collaborate with nursing home staff and participate in team meetings.

    • Supervise and mentor junior social workers and interns.



  4. Documentation and Compliance:

    • Maintain accurate records and ensure compliance with regulations.



Position Requirements

  • Education: Master’s degree in Social Work (MSW) from an accredited program.

  • Experience: Minimum of 5 years of social work experience, preferably in a healthcare setting.

  • Licensure: Valid state license to practice social work.

  • Skills: Strong interpersonal, communication, and organizational skills.

  • Knowledge: Familiarity with elderly care, mental health, and social services resources.

Other Details

  • Reports to: Director of Social Services

  • Working Hours: Monday to Friday, 9:00 AM to 5:00 PM (with occasional evenings and weekends as needed)

  • Employment Type: Permanent
